This might be what I've just experienced from the other side. Entered a pic, then went to play more, skipped a few, came to an exact same caption in the list...

this might explain the upswing of duplicate name games I've noticed that finish around the same time. sometimes they result in two very cool different ideas :)

I was able to make 4 seperate games in the same minute, I just tried to make another one (even though I knew it wasn't possible) but it successfully submitted... so I tried again with another game to see if it works... yup.

Might be Browser/OS related because it only works on the PC at work.
